How Sculptra Body Works
The science of skin ageing on the body
The same biological process that causes the face to age affects every area of the body, yet the body is far less often discussed. From our mid-thirties, collagen production begins to decline. By age 45, the skin may have lost around 25% of its structural collagen. On the body, this manifests as skin that feels thinner, loses its elasticity, and develops a crepey or loosened appearance; particularly in areas such as the upper arms, abdomen, thighs, above the knees, and the décolletage.
Hormonal changes, including the oestrogen decline associated with perimenopause and menopause, accelerate this process. Weight fluctuations and pregnancy can also stretch the skin beyond its capacity to rebound fully, resulting in laxity that is not related to age alone.
Standard moisturisers and topical treatments, however sophisticated, work at the surface. They cannot address the structural collagen and elastin loss occurring deep within the dermis. This is precisely where Sculptra Body intervenes.
What Sculptra Body contains and how it works
Sculptra’s active ingredient, poly-L-lactic acid (PLLA), is a synthetic, biodegradable polymer with a long and well-established safety record in medicine; it has been used in absorbable sutures and orthopaedic implants for decades. In the context of aesthetic treatment, PLLA particles are injected into the deep dermis, where they initiate a controlled biological response.
Research published in the Journal of Cosmetic Dermatology (Zhu & Dong, 2023) demonstrated that PLLA activates the cellular machinery responsible for collagen production at a molecular level. As the PLLA particles are gradually absorbed by the body over several months, new collagen continues to form in their place. The result is a genuine increase in dermal thickness, skin firmness, and surface texture.
For the body specifically, this process has been evaluated in multiple independent clinical studies across different treatment areas:
- Expert recommendations and clinical experience supporting the use of PLLA for skin laxity across non-facial areas, including the arms, abdomen, and thighs, have been published in peer-reviewed literature since at least 2019, with further consensus building consistently since. (Haddad et al., J Drugs Dermatol, 2019; Mazzuco, Dal’Forno & Hexsel, Dermatologic Surgery, 2020.)
- Upper arms: A prospective, randomised, blinded split-arm trial published in Frontiers in Surgery (Neves et al., 2026) evaluated Sculptra specifically for upper-arm skin laxity and contour in 20 adults. By day 90, skin elasticity had increased significantly in the Sculptra-treated arm (p=0.041). By day 120, skin thickness had increased significantly from 1.80mm to 2.70mm (p=0.0039), confirmed by ultrasound imaging. Both patient and physician aesthetic improvement scores showed marked enhancement, with excellent tolerability and no significant adverse events reported.
- Knees: A randomised, split-body, placebo-controlled clinical trial published in Dermatologic Surgery (Kollipara, Hoss, Boen, Alhaddad & Fabi, 2020) demonstrated statistically significant improvements in skin laxity on the PLLA-treated knee compared to the placebo-treated knee, with no significant adverse events. Improvements were sustained at 84 and 168 days post-treatment.
Buttocks and thighs: A prospective, double-blinded, split-body clinical trial (Almukhtar, Wood, Loyal, Hartman & Fabi, 2023) evaluated PLLA for cellulite and skin laxity of the buttocks and thighs in 20 women, demonstrating significant reduction in depression depth, improvement in skin morphology, and improvement in the grade of skin laxity. No significant side effects were found. - Décolletage: A prospective study evaluating PLLA for décolletage wrinkles (NCT05538728) found both protocol volumes to be well tolerated and effective in improving wrinkle severity, as measured by the Galderma Décolletage Scale.
In 2024, an international consensus panel of experts in aesthetic plastic surgery, including specialists from the UK, USA, Brazil, and Switzerland, published a comprehensive evidence review and practical recommendations for injectable PLLA in body treatments, concluding that current data support a meaningful potential benefit for PLLA use across the neck and décolletage, knees, upper arms, abdomen, buttocks, and thighs. What happens during a Sculptra Body treatment?
Your Sculptra Body treatment begins with a full consultation with one of our qualified medical nurses. This is not a box-ticking exercise, it is a genuine clinical assessment of your skin, your concerns, your health history, and your goals. We will take time to understand what you hope to achieve and to explain clearly what Sculptra Body can and cannot do for you, as well as discussing any appropriate alternatives.
At the treatment appointment itself, the treatment areas are cleansed and a topical or local anaesthetic is applied to enhance your comfort. Sculptra is then injected into the deep dermis using a cannula. The number of vials used will be tailored to your individual needs and the areas being treated; this is determined at consultation and will vary between patients.
Most Sculptra Body treatments require more than one session. On average, patients will need two to three sessions, spaced approximately four weeks apart, to achieve their optimum outcome. Results build progressively over the weeks following each session as new collagen is produced. The most noticeable improvements are typically appreciated around two to three months after your final treatment session.
You will be asked to massage the treated areas for five minutes, five times a day, for the first five days following each treatment. This is an important part of the protocol and helps to ensure an even, natural-looking result.

Sculptra Body is particularly well suited to skin that feels crepey or thin, has lost its firmness and bounce, or has loosened after weight changes or pregnancy. It is not a fat-reduction treatment and is not intended to reshape body contour in the way that a surgical procedure would.
Sculptra Body before and after: what results can I expect?
Sculptra Body results build gradually. You are unlikely to notice a dramatic immediate change, although you may see some temporary plumping for the first 24 hours due to the volume of the carrier solution used during treatment, which will subside quickly.
Over the following weeks and months, as new collagen is produced, you should notice a progressive improvement in skin firmness, texture, and quality. Most patients appreciate the most meaningful results approximately two to three months after their final treatment session. Results vary between individuals, as each person produces collagen at their own rate and responds slightly differently to the treatment.
How long does Sculptra Body last?
Results from Sculptra Body are longer-lasting than many injectable treatments because they are the result of genuine collagen production that ages with you rather than the temporary presence of a temporary filler substance. Clinical studies have demonstrated improvements sustained at 168 days post-treatment for knee laxity; please note that this is where the clinical trial ended at this point rather than being indicative of the full longevity. For facial applications, where the longest-term data exists, patients have reported results lasting well over two years. Individual results will vary, and a personalised assessment of expected longevity will be discussed at your consultation.
How many sessions will I need?
The number of sessions required depends on the degree of skin laxity, the number and size of areas being treated, and your individual response to the treatment. Most patients will require two to three sessions, each involving two vials of Sculptra, spaced several weeks apart. This will be determined and discussed clearly at your consultation. There is no one-size-fits-all protocol; your treatment plan will be tailored to you.
How long do Sculptra Body injections stay in the body?
Sculptra’s poly-L-lactic acid particles are gradually, safely, and completely absorbed by the body over the course of several months following each treatment session. By the time they have been absorbed, they will have already stimulated the production of new collagen, which remains in place long after the PLLA itself has gone. Sculptra does not remain in the body permanently.

Is Sculptra Body safe? What are the side effects?
Sculptra has a well-established safety profile across more than two decades of clinical use. As an injectable treatment, you can expect injection-related side effects such as temporary bruising, mild swelling, and tenderness at the treatment sites. These typically start to resolve within a few hours to a few days.
Less commonly, small nodules can form under the skin if post-treatment massage is not carried out as instructed in your aftercare. This is why the five-day massage protocol following each treatment is so important, and why we always take care to explain it fully before your appointment.

Can Sculptra Body be combined with other treatments?
Yes, and for patients with more advanced skin laxity, a combination approach may offer the most comprehensive outcome. Sculptra Body combines particularly well with Thermage, a radiofrequency skin tightening treatment, for example. This pairing complements Sculptra’s deep collagen and elastin biostimulation with Thermage’s surface and mid-level skin tightening action. Whether a combination is right for you will be assessed during your consultation.
I have crepey, loose, or wrinkled skin on my body, is Sculptra Body the right treatment for me?
It may be but the most important step is a proper clinical assessment and consultation. Crepey, loose, or wrinkled skin on the body can have a number of contributing causes, including natural collagen loss with age, hormonal changes, sun damage, and the lasting effects of weight fluctuations or pregnancy. Different presentations can respond best to different approaches.
