The Profhilo Questions We Get Asked Most, Answered Honestly

If you've spent any time researching aesthetic treatments, Profhilo has almost certainly come up - perhaps a friend's recommendation, in a before-and-after post, or in the comments under a celebrity interview. It's one of the most talked-about treatments in aesthetics right now, and for good reason. But with popularity comes noise, and with noise comes confusion. Is it a filler? Is it a facial? Does it hurt? Is it actually worth it?

Here we’ll answer the questions our patients ask most, and we’ll do it honestly, clinically, and without the marketing gloss.

What is Profhilo treatment?

Profhilo treatment is an injectable skin quality treatment made from ultra-pure, thermally stabilised hyaluronic acid (HA). Unlike traditional HA fillers, it contains no added chemical cross-linking agents. Instead, a patented thermal process binds high and low molecular weight HA. When injected, this hybrid of hyaluronic acid doesn't sit in one place or add volume. It disperses through a deep layer of the skin called the dermis, triggering collagen and elastin as it goes.

A peer-reviewed study published in Scientific Reports by Fundarò et al. (2016) demonstrated that Profhilo stimulated increased expression of collagen types I, III, IV and VII, as well as elastin. In other words, Profhilo doesn't just sit in the skin, it sets the skin's own renewal processes in motion all whilst deeply hydrating.

Nurse Natalia tells us: “Profhilo is licensed for use on the face, neck and hands. It is sometimes referred to as a Profhilo face treatment, though its mechanism of action is quite different from anything the word 'facial' implies. Think of it less as a treatment and more as a biological prompt.”

How is Profhilo different from dermal filler?

This is a question we hear so often about the Profhilo treatment, and it matters because the two are genuinely different treatments, used for different purposes, by different types of patients, with different goals.

“Dermal filler adds volume to specific areas. It restores contour or volume to lip borders, cheeks, and jawlines, for example", says Nurse Jane. “The result is structural: you put something back where it has been lost. Profhilo, by contrast, is a skin quality treatment, not a volume treatment. It does not change the shape of your face. It changes the condition of your skin.”

The aim is improved hydration, firmness, elasticity and luminosity - an overall mid-to-lower-face improvement in skin quality rather than targeted correction of, for example, a specific wrinkle. Hyaluronic acid is a molecule found naturally throughout the body where, in the dermis, it plays a central role in binding water and maintaining the structural environment of the skin. Research published in Dermatoendocrinology (Papakonstantinou et al., 2012) describes HA as "the key molecule involved in skin moisture" and confirms that its levels decline significantly with age. Profhilo treatment, often referred to as ‘the injectable moisturiser’, works by replenishing this whilst reactivating skin functions such as collagen production.

Fillers are facial structure, Profhilo is skin quality. Both have their place, sometimes in combination with each other, and sometimes as standalone treatments. They simply serve different needs.

Does Profhilo hurt, and what is the downtime?

Most patients find it very manageable and often more comfortable than they expected.

The injections are placed at five specific points on each side of the face, using a fine needle. This precisely mapped injection technique is known as the BAP (Bio Aesthetic Points) protocol. Each injection delivers a small amount of product to these precise sites, and the whole treatment typically takes around 25 minutes. “There is a brief stinging sensation at the time of injection which our experienced nurses manage well with different techniques to make the experience as comfortable as possible", explains clinic co-founder, Kerri.

Some patients notice small, raised bumps at the injection sites immediately afterwards - in our experience, these tend to settle within a few hours. Bruising is possible, as with any injectable treatment, but tends to be minimal. There is no significant downtime and the majority of our patients return to most normal activities straight away.

A postmarketing surveillance study published on the safety of Profhilo across more than 40,000 patients confirmed a favourable safety profile, with adverse events being predominantly mild and transient. It is not a procedure that requires recovery time, which is part of what makes it so practical for patients with busy schedules.

How many sessions do you need, and how long do results last?

The standard Profhilo treatment protocol is two sessions, four weeks apart. This protocol is the full treatment. Results are cumulative, with the collagen response building progressively between and after sessions.

Nurse Natalia tells us: “Most patients notice a visible difference in hydration and overall skin quality within a few days of the first session. Peak results, once the collagen response has fully developed following the second session, are typically seen after four weeks. The skin continues to improve during this window as new collagen and elastin form.”

In terms of duration, most patients find results last approximately six months, though individual experience varies. “We would typically expect to see someone for Profhilo treatment twice a year", says Nurse Jane.

Factors including skin quality, age, lifestyle and sun exposure all play a role in treatment longevity. Anecdotally, some patients report continued improvement after the 6-month point. As Nurse Jane explained, maintenance sessions every six months is a common pattern for patients who choose to make Profhilo a regular part of their skincare investment.

Is Profhilo worth it?

Our honest answer is, for the right patient, Profhilo is one of the most consistent ways to level-up your skincare regime.

“The results are natural-looking because they are natural. It’s your own collagen, your own elastin, stimulated rather than supplemented", says Nurse Natalia, “Patients consistently describe the improvement as the thing that makes their existing skincare finally feel like it's working harder. The skin looks more awake, more radiant, more itself. Better.”

Whether Profhilo is worth it for you specifically depends on your skin, your goals and where you are in your skincare journey. It is not the right treatment for everyone, and we would rather tell you that upfront than recommend something that isn't the best fit for your skin. Who is Profhilo best for?

In our experience, Profhilo face treatment works particularly well for patients in their early-to-mid thirties through to their mid-forties who are noticing a change in skin quality rather than skin volume - with a shift from vivid to slightly dull, from elastic to a little less resilient. It suits all skin tones and is well-tolerated by sensitive skin. It's especially suited to patients who want a genuinely natural outcome, those who aren't interested in looking changed, but in looking like themselves on a very good day.

“We also see patients who ask specifically about Profhilo under their eyes which is a very common area of concern", says Nurse Jane. “Profhilo is not injected directly into the periorbital area, it is designed to treat the mid-to-lower face only. However, there are a number of data-supported aesthetic treatments that are specifically designed for the delicate eye area, be it undereye, the ‘crows feet’ area, or sagging eyelids.” Such treatments include Thermage eye, polynucleotides, tear trough filler, and SuneKos. Profhilo vs Sculptra: what's the difference?

Both Profhilo vs Sculptra is a comparison that comes up often, because both are classed as biostimulators - treatments that work by stimulating the body's own collagen production rather than adding direct volume. But the two use entirely different mechanisms and they are suited to different patients.

What pairs well with Profhilo?

Profhilo treatment works at the deeper dermal level, stimulating skin quality from within. Surface-level treatments complement this beautifully and the results of addressing both levels together can genuinely be greater than either approach alone.

A mandelic acid peel, such as EnerPeel, the medically licensed formulation we use at the Clinic, accelerates cell renewal at the surface, improving tone, texture and radiance while Profhilo works deeper. The combination addresses skin renewal at every level: collagen and elastin deeper down; surface cell turnover and clarity above. Mandelic acid peels can even be used during the warmer months on all skin tones and even sensitive, rosacea skin.

Other treatments that we’ve experienced complement Profhilo well include HydraFacial, HIFU, Byonik - nearly all of our treatment menu to be honest because when your skin is this hydrated, everything else simply works better. However, the timing of each is something your clinician will advise on during consultation.

What we look for before recommending Profhilo

Not everyone who walks through our door needs Profhilo and we would rather have that conversation than simply recommend it because you’ve asked for it or because it happens to be trending. When we assess whether Profhilo is the right choice for a patient, we're looking for a specific pattern: skin that has changed in quality rather than structure; good volume, but less vitality and hydration; good routine, but less luminosity; skin that feels like it needs refreshing from the inside out, not reshaping from the outside in.

Alongside what you report, we also consider skin condition as we see it, medical history, any active skin concerns, and whether combination treatment or an alternative treatment would give a meaningfully better outcome. This is why a consultation always comes first. It's not a formality, it's where the real result begins.
